How to request a Sacramental Certificate

In accordance with the policies of the Archdiocese of Detroit the sacramental records of Our Lady of Fatima, Oak Park, Michigan, and St. James, Ferndale, Michigan, and Our Mother of Perpetual Help Parish are in the custody of Our Mother of Perpetual Help Parish located at 13500 Oak Park Blvd., Oak Park, Michigan 48237.  248.545.2310

 

NOTE: NO certificates are issued for genealogical purposes. To be valid, certificates must be mailed to you or you may pick them up; however, they cannot be emailed or faxed.

 

In order to protect the confidentiality of these records, certificates will only be issued to the individual named on the certificate, the parent or legal guardian of a minor, or a requesting parish.  Additionally, in accordance with Archdiocesan policy you also need to submit a copy of your driver's license or state identification with your written and signed request form.

 

Please allow 7 business days for the requested certificate to be completed.
