Archdiocese of Detroit Policy
the release of sacramental records is limited
While every individual has a right to a certificate of his or her own sacramental record, no one, with the exception of a parent or guardian asking for the record of a minor child, has the right to another person's record.
If someone has power of attorney for another adult, they may request a certificate but must show the parish personnel the power of attorney before the certificate is prepared.
The only other valid requests would be from another Catholic parish for sacramental reasons or from a Catholic tribunal for annulment reasons. No one else may receive a copy of sacramental certificate.

You must:
Complete the sacramental record request form.
Sign the form
Return the form along with a copy of your driver's license or state identification via United States Postal Service or by dropping the information off to the parish office (NO FAXED OR SCANNED REQUEST MAY BE ACCEPTED)
Mail the form to:

Important Information
In accordance with the policies of the Archdiocese of Detroit the sacramental records of Our Lady of Fatima, Oak Park, Michigan, and St. James, Ferndale, Michigan, and Our Mother of Perpetual Help Parish are in the custody of Our Mother of Perpetual Help Parish located at 13500 Oak Park Blvd., Oak Park, Michigan 48237.
No certificates are issued for genealogical purposes. To be valid certificates must be mailed to you or you may pick them up at the parish office during office hours.
They cannot be emailed or faxed.
